Terms and Conditions

Trust starts with transparency. These Terms explain how HorminaCare works, what’s included in our services, and how we keep you safe while using our platform.

These Terms and Conditions (“Terms”) govern your access to and use of HorminaCare Corp.’s services, website(s), telehealth platform, digital tools, and related offerings (“HorminaCare,” “we,” “us,” or “our”). By accessing or using HorminaCare, you agree to be bound by these Terms.

If you do not agree with these Terms, do not use our services.

HorminaCare Corp. is a federally registered Canadian corporation.

2. Eligibility

You must:

  • be 18 years of age or older,
  • reside in Ontario, Canada, and
  • be able to legally enter into a binding agreement

to use HorminaCare’s telehealth and supplement services. If you are temporarily located outside of Ontario or Canada, you may still be able to access certain non-clinical aspects of our services, such as educational content or tools, but telehealth consultations and medical services are only available to users physically located in Ontario at the time of service.

3. Services Provided

HorminaCare provides:

  • Telehealth consultations with licensed Nurse Practitioners (NPs)
  • Diagnosis, treatment planning, and prescription of medication (dispensed through partner pharmacies)
  • Lab testing, including requisitions and interpretation
  • Personalized hormone-support supplement packs
  • Educational content, articles, and general wellness information
  • Patient portal access (HorminaCare Hub)
  • Digital assessments, quizzes, and tools

All medical care is provided solely by licensed healthcare professionals. HorminaCare’s healthcare providers are responsible for their professional practice within the scope of their licensing.

HorminaCare Corp. itself does not practice medicine.

Services are available only to patients located in Ontario at the time of their appointment.

HorminaCare Corp. is not responsible for any issues arising from inaccurate or incomplete information provided by patients. If a medical error occurs due to incomplete or incorrect data, HorminaCare is not liable for any resulting harm or complications. Patients are responsible for providing accurate, complete, and up-to-date health information.

4. Scope of Care & Limitations

HorminaCare provides a valuable supplement to your healthcare, but it is not a replacement for your primary care provider. You must maintain an ongoing relationship with your primary care provider, as HorminaCare is intended for specific telehealth consultations and supplementary services.

HorminaCare cannot be used for:

  • medical emergencies
  • acute or life-threatening symptoms
  • urgent care needs
  • pregnancy care, fertility emergencies, or newborn care
  • conditions that legally or clinically require in-person assessment

If you are experiencing an emergency, call 911 or visit your nearest emergency department.

By using HorminaCare, you acknowledge the potential limitations of telehealth, including technology-related issues such as poor audio/video quality, internet connectivity, or system outages that may impact the quality of your consultation. While HorminaCare will make
reasonable efforts to ensure reliable service, we are not liable for any disruptions or issues caused by external factors, including those outside of our control. It is your responsibility to ensure you have a stable internet connection and a suitable environment for the consultation.

5. Patient Responsibilities

By using our services, you agree to:

  • provide accurate, complete, and current medical information
  • disclose all medications, supplements, allergies, and conditions
  • follow medical advice unless you choose otherwise
  • use HorminaCare only for lawful and appropriate purposes
  • not seek emergency care through the platform
  • not misuse messaging, booking tools, or clinical support
  • maintain the confidentiality of your account login information

Failure to comply may result in suspension or termination of your account. If you repeatedly refuse medical advice or provide false information, we may permanently terminate your account and deny access to future services.

6. Medication & Pharmacy Services

Prescriptions may be issued by licensed Nurse Practitioners (NPs) when deemed medically appropriate. Medications are dispensed through independent partner pharmacies, which operate separately from HorminaCare.

Prescriptions may be sent to Pillway, a licensed Canadian pharmacy, for dispensing and delivery. Patients may choose to have their prescription sent to a different pharmacy of their choice in Ontario, by informing their Nurse Practitioner during the appointment.

HorminaCare is not responsible for:

  • pharmacy dispensing practices
  • shipping delays
  • insurance coverage or claims
  • medication shortages

For any medication-related inquiries or concerns, please contact the pharmacy directly. HorminaCare is not liable for prescription errors unless directly caused by the negligence of one of our healthcare providers.

7. Supplement Services

HorminaCare offers personalized hormone-support supplement packs provided by Persona Nutrition, an independent third-party provider. Please note that:

  • HorminaCare does not manufacture, package, or ship supplements, and therefore is not responsible for their quality, safety, side effects, or fulfillment.
  • All supplement orders are subject to Persona Nutrition’s terms of service, refund policies,
    and safety protocols.
  • Supplements are not a substitute for medical treatment or prescribed medications.

Before using any supplements, you should consult with your primary healthcare provider to ensure they do not interfere with any existing medications or health conditions.

If you experience adverse reactions, discontinue use immediately and consult a medical professional.

8. Appointment Fees, Billing & Payments

Payments are processed securely through Stripe.

8.1 Appointment Fees

You agree to pay all fees associated with:

  • Telehealth appointments
  • Lab review appointments (if applicable)
  • Additional services as listed during booking

8.2 Cancellation Policy

To ensure fairness to both patients and our healthcare providers, the following cancellation policy applies:

  • Appointments cancelled 48 hours or more before the scheduled time: fully refundable
  • Appointments cancelled within 48 hours: non-refundable
  • No-shows: non-refundable

8.3 Payment Authorization

By booking an appointment, you authorize HorminaCare to:

  • charge your card for applicable fees
  • store your payment method securely for future transactions

If you cancel an appointment within the allowed time frame, any fees paid will be refunded excluding charges for services such as prescriptions or supplements, which are non-refundable once processed.

9. Digital Tools, Quizzes & Educational Content

HorminaCare provides educational material, assessments, quizzes, and wellness information for general informational purposes only.

These tools:

  • do not constitute medical advice
  • do not establish a clinician-patient relationship
  • may not reflect your complete medical picture

Decisions about your health should always be made in consultation with a licensed healthcare professional.

10. Intellectual Property

All content on HorminaCare’s website and platform — including branding, text, graphics, videos, quizzes, algorithms, assessments, and the HorminaCare name — is the intellectual property of HorminaCare Corp.

Users may not:

  • copy
  • distribute
  • reverse engineer
  • modify
  • impersonate
  • commercially use any part of the platform or brand

Violations may result in account termination and legal action. Any content provided by third-party partners, including supplement information or educational resources, is subject to their respective intellectual property policies.

11. Privacy & Health Information

We are committed to the protection of your personal health information. By using HorminaCare, you consent to: the collection, storage and processing of your personal and health information as outlined in our Privacy Policy.

HorminaCare follows strict protocols to ensure your data is secure and stored in compliance with Canadian privacy laws, including the Personal Information Protection and Electronic Documents Act (PIPEDA).

Your health data is stored in EMERGE, our secure Electronic Medical Record (EMR) system, which adheres to industry standards for health information security and confidentiality.

You consent to receiving electronic communications related to your care, including reminders, updates, and informational materials.

12. Service Availability

We do not guarantee:

  • uninterrupted or error-free service
  • availability of specific healthcare providers
  • availability of specific appointment times
  • accuracy of third-party content or systems

We may suspend or modify the platform at any time for maintenance, updates, or operational needs. HorminaCare is not liable for any damages or losses caused by service interruptions. However, we will strive to restore services as quickly as possible.

13. Liability, Warranties & Disclaimers

HorminaCare is provided “as available”. We make no guarantees about the continuity or quality of service, and we do not guarantee specific medical outcomes or improvements.

You acknowledge that:

  • HorminaCare is not responsible for delays, inaccuracies, or issues caused by third-party pharmacies, labs, supplement providers, or technology platforms
  • To the extent permitted by law, HorminaCare’s total liability will not exceed the amount you paid for the service at issue

You agree to use your clinical judgment when using HorminaCare’s services and seek in-person care when appropriate.

You agree to release HorminaCare from liability for issues resulting from inaccurate information you provide.

You agree to indemnify and hold harmless HorminaCare, its affiliates and its healthcare providers from any claim, loss, or damages arising out of your misuse of the platform or violation of these Terms.

14. Dispute Resolution

HorminaCare is committed to resolving disputes in a fair and timely manner. Before initiating any legal action, you agree to attempt to resolve disputes through binding arbitration conducted in Ontario, Canada.

Arbitration will be conducted according to the rules of a recognized arbitration body, such as the Ontario Arbitration Association. You waive the right to participate in class actions or class arbitration.

Arbitration decisions are final and enforceable.

15. Governing Law

These Terms are governed by the laws of the Province of Ontario and the applicable federal laws of Canada.

16. Termination

HorminaCare reserves the right to suspend or terminate your account for violations of these Terms, including but not limited to:

  • You misuse the service
  • Providing false or misleading medical information
  • Repeated refusal to follow medical advice

You may terminate your account at any time by contacting us. If your account is terminated, you may request a review of the decision and we will assess whether reinstatement is appropriate.

17. Changes to These Terms

We may update these Terms from time to time.
Continued use of the platform after updates constitutes acceptance of the revised Terms.
